Palliative and end of life care factsheet:
Recent trends in place of death

NHS Cheshire and Merseyside Integrated Care Board

Published January 2026

Background

Place of death statistics describe where people die (hospital, home, care home, hospice and other places) and how that is changing. Patient preference and policy is for people near the end of life to be cared for in their own home or care home and to reduce unwarranted emergency hospital admissions at the end of life.

Key messages

Approximately half of people are at home or in a care home when they die

In England, 50.3% of people were at home or in a care home when they died (12 months from November 2024 to October 2025). Approximately two fifths 41.4% died in a hospital and 5.7% died in a hospice.

In Cheshire and Merseyside 12,845 people died at home or in a care home (47.8%, lower than England), 12,210 people died in hospital (45.4%, higher than England) and 1,244 people died in a hospice (4.6%, lower than England).

During the COVID-19 pandemic there were abrupt changes in where people die. More people died at home and this change has continued

Previous analysis showed a marked increase in the percentage of people who die at home during the COVID-19 pandemic. This increase has been maintained over the last three years. In England during the latest 12 months from November 2024 to October 2025, 149,032 people died at home (28.1%).

In Cheshire and Merseyside, 7,402 people died at home in the latest 12 months (27.5%), a median average of 601 per month. This is lower than the percentage in England.

  • the comparison to England is made by calculating 95% confidence intervals for the ICB value and comparing whether they overlap the England value (the reference value)

  • the Wilson Score method was used to calculate the confidence intervals

  • the Wilson Score method gives very accurate approximate confidence intervals for proportions and odds based on the assumption of a Binomial distribution. It can be used with any data values, even when the denominator is very small and, unlike some methods, it does not fail to give an interval when the numerator count, and therefore the proportion, is zero. The Wilson Score method is the preferred method for calculating confidence intervals for proportions and odds, but it can also be used for rates, as long as the event rate is low (relatively rare events within the population) as the Binomial distribution is a very good approximation to the Poisson distribution when the event rate is low. The method is described in detail in APHO Technical Briefing 3: Commonly used public health statistics and their confidence intervals.

  • throughout this factsheet the month or year a death is reported is determined by the date of registration, there can be a delay between the date a death occurred and the date a death was registered. This is explained by the Office for National Statistics (ONS) in Impact of registration delays on mortality statistics

  • individual deaths are assigned to a ICB according to the postcode of the deceased’s place of residence. Data is summarised into ICBs as configured in December 2024

  • the reported ICB is not necessarily the ICB where a death occurred

  • place of death, and place of residence are recorded in the mortality data. For further information on the methods used for all place of death indicators as classified by ONS death certificate data, see Classification of Place of Death: A technical bulletin

  • coding of sex, ‘Unknown’ is not included where data are broken down by sex, but is included for Persons, terefore the sum of Male and Female might not equal Persons

  • the reported sex for each deceased person is that supplied by the informant to the registrar when the death is registered.

  • percentages are rounded to one digit, consequently they may not total exactly 100%

Local considerations

Commissioners and service providers should review the data presented in this factsheet alongside several other data and intelligence tools and reports. They should also consider using locally produced data on specialist palliative care and social care services. We have provided information on the main supporting resources in this section.

When interpreting data on end of life care in the community commissioners and service providers should consider:

  • how do these levels of identification compare with the demographic and disease profiles in each place?

  • does the data reveal inequity of identification based on age, diagnosis, ethnicity, deprivation or other factors?

  • can the levels of identification and offer of personalised care and support planning be improved?

  • what lessons can be shared by those places with high levels of identification and conversations offered?

  • is there adequate support for unpaid carers involved in looking after the person at home?

When interpreting data on end of life care in care homes commissioners and service providers should consider:

  • is generalist and specialist palliative care provision for care homes adequate, in and out of hours?

  • does this meet the needs set out in the Enhanced Health in Care Homes framework?

  • is there adequate provision for education, training and support for care home staff?

  • are arrangements for rapid access to medicines and equipment, including syringe drivers sufficient?

When interpreting data on end of life care in hospitals commissioners and service providers should consider:

  • do hospitals have adequate arrangements for recognising when someone is likely to be in the last year of life and also when someone is imminently dying?

  • how do hospitals ensure that all their clinical and support staff have adequate training to support people near the end of their lives?

  • is there access to specialist palliative care advice any time of day or night?

  • what arrangements do hospitals have for ensuring high quality PEoLC for the person, including care after death, and for those important to them and for bereavement?

Local indicators on place of death on the Palliative and End of Life Care Profiles. These include breakdowns by different age groups and can be found in the place of death topic section.

The Patterns of Care Factsheet describes patterns of care near the end of life related to four major conditions: cancer, cardiovascular disease, dementia and respiratory disease. It also includes experimental statistics describing: the number of people who died, their cause of death, demographic characteristics, hospital admissions, length of stay in hospital and place of death.

The Care Home Factsheets for ICBs describe data on the current provision and future need for end of life care in care homes. They also provide contextual data on care home beds, staff capacity, and training of care home staff.

Sources of place of death data in Scotland, Wales and Northern Ireland are described in Coherence of end of life care statistics in the UK
